We provide person centered, structured support to individuals with intellectual and/or developmental disabilities by offering residential and day services within our Family Lifestyle Model (similar to foster parent responsibility). Our person centered approach empowers individuals to lead their best, most independent lives through individual driven care.
In the Family Lifestyle Model, contracted families (Family Lifestyle Teachers) provide services in their home to adults that have an intellectual or developmental disability as a primary diagnosis. We match individuals with their families based on preferences, interests, routines, and support needs. This helps ensure a stimulating, engaged lifestyle for the individuals supported and fosters long-lasting stability in those support relationships.
Our Family Lifestyle Teachers are paired with Lifestyle Coaches, who are equipped with a structured curriculum designed to ensure that Family Lifestyle Teachers are able to demonstrate important operational procedures. The Family Model Homes are also monitored in critical skill areas that are specific to each individual served in their home.
The success of the Family Lifestyle Model is very much dependent upon our fantastic family teachers (similar to foster parents) and our talented administrative group. We attribute much of their success to the model’s foundation, wherein families develop consistent and strong relationships with the individual(s) living in their home. When individuals are paired with caregivers, they are able to advance towards their own goals with consistent, loving support from those they live with.
Caring for an adult with disabilities is a full time job. Family Model Homes care for an individual in their home 24/7, 365 days a year, with support from our therapeutic foster care specialists. After completing your training during the hiring process, you will wait to be matched with an individual who you are compatible with. The placement of an adult with disabilities in your home and care may be immediate, or may take months, depending on the individuals currently seeking care in Tennessee.
